NEW FOR THE 2005/2006 SCHOOL YEAR

We need parents to participate in our Health Program, to help develop plans
for a walking program and nutritional improvements for our students. If you are interested in helping us with this program, please contact:

FOR YOUR INFORMATION

1. Health Information on your student must be updated each school year. If your child takes medication at school, you must fill out the medication authorization form and have your physician fill out his portion of the form and sign it, then return the signed form to the school. This is the only way medications can be given by school staff at school. This is for prescription medications and Nonprescription medications bought over the counter.

2. Medications cannot be transported to school by students. Parents/Guardians must bring the medication to the school and give it to the school staff. Many medications are controlled substances and must be counted by the parent/guardian with the staff when they are brought to the school and when medication is returned to take home.

3. If your student has Asthma and the doctor has ordered an inhaler to be used at school, please pick up the correct form from the school staff for you and your physician to sign. If your physician wants your child to carry their own inhaler with them at school, this form must be filled out, signed, and returned to the school staff. Thank you for helping us provide for the health needs of your student while they are in school. COUNTY HEALTH DEPARTMENT.


ALABAMA HEALTH INFORMATION

REMEMBER TO CHECK YOUR CHILD'S
IMMUNIZATION RECORDS
TO MAKE SURE THEY ARE UP-TO-DATE AS
REQUIRED BY THE STATE.

ALL STUDENTS ENTERING PUBLIC OR PRIVATE SCHOOL
IN ALABAMA ARE REQUIRED TO PRESENT AN ALABAMA
CERTIFICATE OF IMMUNIZATION (IMM-50) OR EXEMPTION
BEFORE ATTENDING SCHOOL AT ANY GRADE LEVEL.

REMEMBER: YOUR CHILD ALSO NEEDS TO HAVE HAD
(2) VACCINES FOR MEASLES, MUMPS AND RUBELLA (MMR).
THIS NEEDS TO BE DOCUMENTED ON YOUR HEALTH
CERTIFICATE. IF YOUR CHILD IS IN GRADES K-3,
THEY MUST SHOW PROOF THAT THEY HAVE HAD THE
VARICELLA VACCINE BEFORE ENTERING SCHOOL.

ASK YOUR CHILD'S PRINCIPAL ABOUT AN APPLICATION.

IF YOU DO NOT HAVE INSURANCE, YOU MAY
QUALIFY FOR THIS INSURANCE TO COVER
YOUR CHILD'S HEALTH NEEDS.
