Parents: When you are enrolling your student, please make sure your child has a "blue card" from the Alabama Health Dept. stating that all immunizations are up-to-date.

• 4 DTP's with one dose given after age four.
• 4 polio's but if one is given after age of four, then only 3 are required.
• 1 mumps, 1 rubella, but (2) measles
• Varicella, through 7th grade this year. By 2013 every student must have one.
