attend the field trip below. I understand that my
child will be in another classroom during the day.
During the pupil’s absence I hearby absolve the school, its teachers and other officials, the board of education, and the superintendent from all responsibility for his/her safety because of unavoidable accidents. I certify that I have hospitalization insurance or that the student has been covered under the School Accident Insurance Program.
Please turn in permission forms expeditiously.
