Substitute Teaching Employment

All persons interested in substituting for the Phenix City Board of Education will need to provide the following items. Please print these forms and have them completed when you come to the Board of Education:

Substitute Application Form. No applicant will be intereviewed until three references are on file.

20.00 postal money order or cashier's check payable to the State Department of Education for a Substitute Teacher's License (You will need either a Substitute Teacher's License or a Teaching Certificate)

$49.00 postal money order or cashier's check payable to the State Department of Education for a background check.

Money Orders or cashier's checks for Substitute Teacher's License and background check must be separte and are non-refundable.

Application for Substitute Teacher's License. (This form will be completed at the Board of Education Offices)

Release form for fingerprint processing. (This form will be completed at the Board of Education Offices)

Background check consent form. (This form will be completed at the Board of Education Offices)

Fingerprint cards can be obtained at the Board of Education Office.

Social Security card (card must be signed to be valid)

Driver's License

Employment Eligibility Verification Form . (This form will be completed at the Board of Education Offices)

W-4 & A-4 Forms. (These forms will be completed at the Board of Education Offices)

T.B. Skin Test (no more than 1 year old). This test can be performed at the Russell County Health Department.

Copy of College Diploma, High School Diploma or G.E.D.
